Discharge Medicines Review & Support Service
An online or telephone service whereby we review any discharge medication issued by your hospital.
If yourself or any family members have been disharged from hospital recently, please let us know. We can access your discharge records (which can be very detailed) and check them against new prescriptions issued by your GP. This safely ensures that your GP practice will prescribe according to the doctors in the hospital, and not overlook anything.
